IBX teams up with Isaac Health on a new way to support brain health

Independence Blue Cross (IBX) knows that the brain is at the center of everything a person does, and that keeping it healthy is essential to their well-being. Keeping this in mind, IBX has teamed up with Isaac Health. Isaac Health is a virtual-first clinic that provides brain health and dementia care. Through this new relationship, a subset of IBX Medicare Advantage members who have a diagnosis of dementia or who are at risk for dementia have access to Isaac Health’s services.

“We want to help members who have a diagnosis of dementia address potential brain health concerns early and be connected with the appropriate resources for support,” said Dr. Luz Ramos, geriatrician and medical director at Independence Blue Cross. “Working with Isaac Health helps us empower our members to take charge of their brain health and overall well-being.  This is especially important, as the National Institutes of Health estimates that the aging of our population in the U.S. is expected to cause the number of new dementia cases per year to double by 2060.”

Isaac Health offers a full range of services for people living with dementia and other brain health conditions. The services are intended to help improve quality of life for members and their caregivers. The services that are included are:

  • Comprehensive brain health assessments and consultations with dementia specialists.
  • Neuropsychologist testing and lab imaging.
  • Personalized care plans that may include cognitive therapy and cognitive behavioral therapy.
  • Medication management.
  • Follow-up appointments with a team of neurologists, geriatricians, psychiatrists, and psychotherapists.
  • Skills training to help maximize patients’ independence.
  • Support groups and referrals to a network of community resources.
  • Access to caregiver resources, as well as a dedicated caregiver hotline that is staffed by trained professionals.

“We’re proud to partner with IBX to bring high-quality, accessible dementia care to more people and families who need it,” said Dr. Julius Bruch, CEO and co-founder of Isaac Health. “Through this program, we’re creating a comprehensive support system that helps patients, and their families navigate the complexities of dementia with expert guidance and personalized support. Together with IBX, we’re making it easier for members to take control of their brain health and access the right care at the right time.”

Isaac Health’s services are designed for all who need it. For those with internet access, consultations will take place via telehealth. For those without internet, Isaac Health can arrange for an assistant to visit their home to help facilitate a virtual appointment.

IBX Medicare Advantage members who are eligible can be referred to Isaac Health a few different ways. They can get a referral from their primary care doctor or specialist. Another way to get access is if they have worked with IBX Case Management because of a diagnosis of dementia or are at risk for dementia. In this case, Isaac Health’s specialized team may reach out to them directly.

During outreach calls, Isaac Health specialists will introduce themselves as calling on behalf of IBX. They will also verify each member’s identity for privacy and security purposes. Then they will ask a brief series of questions to determine if additional support or assessments are needed. If a member demonstrates cognitive changes in two or more areas during the initial assessment, they will have the option to schedule a follow-up virtual consultation with an Isaac Health provider.

Members with questions about IBX’s work with Isaac Health can call 888-818-2059 or visit www.myisaachealth.com/IBX.
