An older patient has a conversation with his physician. She is showing him something on a tablet.

As people live longer, health care must adapt to meet their evolving needs. A new approach called age-friendly health care was developed to help people stay active, independent, and supported as they age.

People are more than just their health conditions. Your doctors should support all parts of your well-being. Age-friendly health care focuses on your needs, goals, and daily life. It can help you feel respected, listened to, and supported. It can also help make your life easier to manage.

The “4 Ms” of age-friendly health care

At the center of age-friendly health care is a simple framework called the 4Ms: What Matters, Mobility, Mentation (Mind), and Medication.

  • What Matters: Your care starts with what’s most important to you, like your goals, values, and what you want your life to look like.
  • Mobility: Your care team helps you stay active and steady so you can stay independent and keep doing the things you enjoy.
  • Mentation (Mind): Your mental health is an important part of your overall health, so your care includes support for your memory, thoughts, and mood.
  • Medication: Your care team reviews your medications often to make sure they are safe, work well together, and are still right for you.
Graphic titled: Age-Friendly Health Care Four sections with illustrations of people. 1) What Matters Know and align care with each older adult's specific health outcome goals and care preferences including, but not limited to, end-of-life care, and across settings of care. 2) Medication If medication is necessary, use Age-Friendly medication that does not interfere with What Matters to the older adult, Mobility, or Mentation across settings of care. 3) Mentation Prevent, identify, treat, and manage dementia, depression, and delirium across settings of care. 4) Mobility Ensure that older adults move safely everyday in order to maintain function and do What Matters.

Here is what the 4Ms can look like in real life:

  • What Matters: A doctor is seeing a patient named Mr. Lewis. She asks him what’s most important to him. He says he wants to keep gardening and spending time with his family.
  • Mobility: Knowing this, the doctor checks his balance and strength. She suggests simple stability exercises and ways to make his home safer.
  • Mentation: She asks Mr. Lewis about his memory, mood, and sleep (making sure he feels comfortable sharing about these things).
  • Medication: She reviews all his medicines, including over-the-counter items. She looks out for side effects and explains how to take all these medications safely.

The 4 Ms help Mr. Lewis stay active and focused on what matters most to him.

A growing commitment to age-friendly care

Many older adults deal with more than one health condition. They may also face changes in their mobility, memory, or daily life. Across the country, health systems are adopting age-friendly care to better support these needs.

At Independence Blue Cross (IBX), we’re working with national providers to support this approach. IBX has been designated as a Pioneer Age-Friendly Medicare Advantage Plan, in recognition of our commitment to providing better care for older adults.

How to use the 4Ms at your next appointment

You don’t have to wait for your provider to bring up the 4Ms. You can start the conversation. At your next visit, you can ask:

  • How can my care better match my goals? (What Matters)
  • What should I do to stay active and independent? (Mobility)
  • Are all my medicines still safe and necessary? (Medication)
  • Can we discuss some concerns I have about my memory or mood? (Mentation)

These questions can help you and your care team stay on the same page.

Where caregivers fit in

If you’re caring for an older adult, here’s how you can help:

  • Watch for changes in their mood, memory, or movement.
  • Go to doctor visits with them and ask questions.
  • Share what matters most to your loved one.
  • Write down important information.
  • Help create a safe home.
  • Talk about their future care wishes.
  • Take care of yourself, too.

Caregiving can be hard. Support is available. If your loved one has an IBX Medicare Advantage plan, Carallel offers help and resources at no cost.

Maximizing quality of life

Age-friendly health care is helping change the way we think about aging.

By focusing on the 4Ms, care teams can provide safer, more thoughtful care. At the same time, patients can take a more active role in their health. The 4 Ms help make sure care is not only effective, but also meaningful to you.
